LIBRARY/MEDIA CENTRE ASSESSMENT QUESTIONNAIRE for ELEMENTARY SCHOOLS: How does your library “stack up?” Do you know how your child’s school library compares to others? And more important, does it meet the Standards for School Library Programs set by the Canadian Association for School Libraries? This questionnaire was designed to help you find out. It is a condensed version of information found in the publication, Achieving Information Literacy: Standards for School Library Programs in Canada. For further information on this and other documents related to school libraries, please go to www.ontarioschoollibraries.ca You may need to get input from your school principal or teacher-librarian in order to fill in this form accurately and completely. For each question, mark the appropriate column. Then add up the total number of points scored to compute your school library’s overall rating.
